WHAT "PRE-EXPERIMENTATION" DOCUMENTATION IS NEEDED?

ELEMENTARY SCHOOL PROJECTS

For students in grades 5 to 8, the HUMAN SUBJECT APPROVAL CERTIFICATE found in the "FORMS" section of this website MUST be completed IN ADVANCE of the commencement of the research work. Upon completion the form should be put aside until the students make application to the NRSEF, at which time it should be fastened securely to the signature forms.

NOTE: If human subjects were involved, even "question and answer" you need a Human Subject Approval Certificate.

SECONDARY SCHOOL PROJECTS

ALL secondary school wishing to enter a project to the NRSEF must obtain and complete the "Teacher Supervisor / Research Plan / Safety Review Certificate Form". This form must be completed, signed by the Teacher or Supervisor, and DATED BEFORE beginning research, and is to be included with the signatures forms to the NRSEF.

For all projects involving vertebrate animals, humans, recombinant DNA, tissue or pathogenic agents, or controlled substances, ADDITIONAL certificate forms must be obtained and completed. Each of these forms, as well as the Certification Form mentioned above, must be submitted with the signature forms to the NRSEF.
